Bromley Connect PCN formed in 2019 and comprises of 3 general practices in Bromley - South View Practice, Dysart Surgery and London Lane Clinic.

The practices have a combined geographic area made up of around 40,000 patients.

There is a wide socio-economic and demographic served within this area which will provide an interesting and varied case mix. We pride ourselves on offering our patients the best care possible by working in collaboration with our allied health care professionals across the network. Bromley Connect PCN support in-house training of our additional roles staff. By working together, we are able to offer our services at times that may be more flexible for our patients across the PCN.

What can you expect from your PCN

Additional healthcare professionals have been recruited to work on behalf on Primary Care Networks.  New roles include Care coordinators, Clinical Pharmacists, First Contact Physiotherapists, Mental Health professionals and Social Prescribing Link Workers. By introducing a greater skill mix, local people will get more access to the support they need to keep people healthy and independent.
